All students whose parents indicated on their enrolment form that a medical condition is present should have received an information letter and a series of forms for completion and return to school.
All other parents, please also access these files if your child has a new medical condition or one not currently known to the school.
It is preferred that soft copies are completed and emailed back to mowbray-p.school@det.nsw.edu.au, however hard copies are also accepted. Soft copies will be available on the school website too.

If your child has a health condition, please download this form and hand it into the office to request for support at school of a student's health condition (DOC 59.7KB).
Download the individual health care plan form (DOC 59.9KB) and hand it into the office.
If you allow the school to contact a doctor on your behalf, please download the parent consent to contact doctor (DOC 40.9KB) form and hand it into the office.
